Clinical description

TOSCA® is a plate-and-screw system for anterior (ventral) cervical stabilization of the C2–C7 segment, developed by SIGNUS. The pre-lordosed plate is available in various lengths, for constructs from one to four segments, and is adapted to the natural lordosis of the cervical spine — so that, in most cases, it can be implanted without additional bending. With a thickness of only 1.6 mm, TOSCA® is one of the thinnest plates on the market.

The semi-rigid plate–screw connection is the defining principle: it reduces the stress-shielding effect and allows optimal load transfer to the bone (Wolff’s law), supporting fusion of the segment and physiological subsidence ("graft settling"). The self-tapping screws do not require pre-tapping and have an angular variability of 5° in all directions. The system is made of biocompatible titanium alloy (Ti-6Al-4V).

TOSCA® is available in two fixation variants. The Standard variant uses standard variable-angle screws (Ø 4.0 / 4.5 mm, lengths 12–26 mm), allowing mono- or bicortical anchorage. The Expansion variant adds expansion screws, in variable-angle and fixed-angle versions, which provide primary stability comparable to that of bicortical screws but without the risk of dural injury — useful in poor-quality bone and in long constructs. With a connecting screw, TOSCA® can be fixed to the SIGNUS cervical vertebral body replacement implants (ATHLET®, PALIO®) and to the disc cages (RABEA®, NUBIC®, JASPIS® ST), as well as to a bone graft, for a secure construct.

  • Trauma; tumor; deformity.
  • Pseudarthrosis or absence of fusion.
